BY SCHEDULING AN APPOINTMENT OR RECEIVING SERVICES AT ONE DENTAL SPECIALTIES, YOU AGREE TO THE FOLLOWING TERMS. PLEASE REVIEW THEM CAREFULLY.
1. Services
One Dental Specialties provides dental and prosthodontic services at 621 S. Western Ave. #201, Los Angeles, CA 90005. All treatment is subject to a clinical evaluation and the professional judgment of our providers. We reserve the right to decline or discontinue services at our discretion.
2. Appointments and Cancellation Policy
We ask that you provide at least 48 hours notice if you need to cancel or reschedule your appointment. Late cancellations or missed appointments without prior notice may result in a cancellation fee. Repeated no-shows may result in discharge from the practice.
3. Payment and Fees
Payment is due at the time services are rendered unless prior arrangements have been made. We accept cash, major credit cards, and approved financing options. Fees are subject to change. You are responsible for all charges incurred during your visit, including any balances not covered by insurance.
4. Insurance
We will assist in filing claims to your dental insurance provider as a courtesy. However, you are ultimately responsible for any balance not covered by your plan, including deductibles, co-pays, and non-covered services. We are not responsible for errors or denials made by your insurance carrier.
5. Patient Responsibilities
You agree to provide accurate and complete medical and dental history at the time of your visit and to update us of any changes. You are responsible for following post-treatment care instructions provided by your provider. Failure to do so may affect your treatment outcomes, for which One Dental Specialties bears no liability.
6. Informed Consent
By proceeding with any treatment, you acknowledge that you have been informed of the nature of the procedure, potential risks, and available alternatives, and that you consent to the recommended care. Separate written consent forms may be required for certain procedures.
7. Privacy
Your health information is handled in accordance with our Notice of Privacy Practices (NPP), available upon request and at onedentalspecialties.com/accessibility. We comply with all applicable HIPAA regulations and California privacy laws.
8. Limitation of Liability
One Dental Specialties is not liable for any indirect, incidental, or consequential damages arising from treatment, delays in treatment, or reliance on information provided at our practice. Our liability is limited to the cost of the services rendered.
9. Changes to These Terms
We reserve the right to update these Terms of Service at any time. Continued use of our services following any changes constitutes your acceptance of the revised terms.
